Monthly Commitments
To support my ministry on a monthly basis please fill out this Commitment Form and fax it
to the Assemblies of God World Missions Office at the number on this form. My AGWM
Account Number is 293680. I have already filled in my information on this form. Monthly
statements of your YTD contributions with a return envelope will be sent to you; these are
your tax-deductible receipts. I receive the appropriate disbursement of your contributions
approximately the 10th of each month.

This link will take you to my secure online donations page in the A/G website; the web
security is hosted by Secure 1. The ONLY payment options this way are Visa, Mastercard
and Discover. There is a receipt page at the end for you to print for personal and tax
records. There is a Monthly Draft option available through this link or you can click here
to print and fax it or mail it in.
Checks - You can contribute to my ministry with a personal, corporate, or church check.

Once AGWM has received your contribution a receipt statement will be mailed to you to
keep for your tax records. A detachable slip will be included for your convenience for
